Volunteer recruitment for the season and annual registration takes place following each orientation session. Parents should go to the Fine Arts Lobby & Sjolund Auditorium at North Medford High School (NMHS) on Sunday, October 5.

The Youth Symphony of Southern Oregon parent orientation sessions in the fall offer an opportunity for parents to become acquainted with each other, meet staff and members of the governing board, submit all required musician registration forms/releases and payments, volunteer for tasks and duties during the season, and learn about the organization.

Forms, releases or payments not turned in at registration can be mailed to the YSSO. The mailing address is: YSSO, P.O. Box 4291, Medford, OR 97501.

Forms are "fillable" PDFs. Save the individual form or packet of forms to your computer. Type responses into the fields on each form. Once the forms are complete, print and then sign where indicated on the forms/releases. Forms can be printed and completed by hand as well.

Click the link above for a packet of fillable PDF forms/releases including general contact information form, tuition form, risk assessment & liability release, medical permission and media release form, parent and musician contract, school music program participation verification form, and volunteer checklist. All musicians must submit completed and signed forms/releases each year to participate and to be listed in concert programs.

Concerto Competition Intent to Participate Form (fillable PDF)
Optional. The Concerto Competition is for eligible, advanced-level Youth Symphony musicians. Eligible applicants must complete and turn in the form with all registration/release forms, payments and Concerto Competition fee if interested in participating in the competition. Please review the information on the form to confirm eligibility. Late Concerto Competition Intent to Participate forms will not be accepted.

Chamber Music Series Application Form (fillable PDF)
Optional. The Chamber Music Series program is intended for advanced-level Youth Symphony musicians. The Chamber Music Series form must be completed and turned in at registration with all other required registration forms/releases. All registration forms/releases and any applicable payments/fees must be turned in to be eligible for consideration for participation in the Chamber Music Series and the Concerto Competition.

2025-2026 Tuition Assistance Request Form
Contact the Youth Symphony of Southern Oregon by e-mail (info@ysso.org) to request this one-page form. In order to be considered, the tuition assistance request form must be completed and turned in with all completed, signed registration/release forms at registration. Please review the instructions and provide all of the information requested.

NOTE: The YSSO uses photographs, recordings, and other materials for publicity and promotional purposes and online. All YSSO musicians and families must complete and sign the Media, Recording & Photo Release and submit the form with all other annual registration forms/releases.
