Registration, Parent Orientation, Forms & Payments

The Youth Symphony of Southern Oregon organization holds Parent Orientation Sessions in the fall for parents to become acquainted with each other, meet the conductors, staff and governing board, submit all required registration forms/releases and payments, volunteer for tasks and duties during the season, and learn about the organization. YSSO parents, families and musicians are expected to understand and comply with all YSSO organization policies, procedures, expectations and schedules.

Parents are asked to sign up for a variety of important volunteer tasks following the orientation sessions. Volunteer coordinators will be on hand to schedule volunteers. YSSO families are asked to assist with a minimum of at least two volunteer activities during the season. The success of the Youth Symphony of Southern Oregon is dependent upon the support and active participation of parents and families.

Parent Orientation Sessions and Registration for the 2017-2018 Concert Season took place at North Medford High School on Sunday, September 24, the day of the first rehearsals. All parents or guaradians were asked to attend the session(s) and registration. Youth Orchestra parents met at 1:00 p.m. Youth Symphony parents met at 3:00 p.m. The registration and volunteer recruitment tables opened following each orientation session.

All completed, signed forms/releases and payments were due at registration, including the School Music Program participation form.

2017-2018 Required Registration Forms/Materials Packet (multi-page fillable PDF)
The PDF packet includes a checklist, the registration and tuition forms, medical/permission and media releases, parent/musician contract, school music program participation form, volunteer checklist, and contribution/gift forms. The PDF may be saved to your computer, completed, printed and signed (please sign where indicated on each form/release), or it can be printed and completed by hand. All musicians must submit completed and signed forms/releases to participate and to be listed in concert programs.

Chamber Music Series Application Form (fillable PDF)
The YSSO organization is celebrating its 30th Anniversary in 2017-2018. One of the special events is a Youth Symphony & Youth Symphony Alumni Concert. This concert will take place in December 2017 instead of the Chamber Music Series. The Series is expected to return in 2018. (The Chamber Music Series is optional and it is intended for advanced level YSSO musicians. When the Chamber Music Series returns, the form must be completed and turned in at registration with all other required registration forms to be considered for participation. Please be aware that YSSO musicians must be registered and all registration/release forms and any applicable payments/fees must be turned in to be eligible for consideration for participation in the Chamber Music Series, when offered, and the Concerto Competition.)

(Optional. For eligible advanced Youth Symphony musicians. Eeligible applicants may complete and turn in the form with all required registration/release forms, payments and Concerto Competition fee if interested in participating in the 2018 competition. Please review the information to confirm eligibility. NOTE: There is a receipt deadline for the Intent to Participate form each year. (Late "Concerto Competition Intent to Partcipate" forms will not be accepted.)

YSSO musicians/families must complete, sign and turn in all registration forms and submit applicable payments/fees in order to participate and to be listed in concert programs as well as to be eligible for consideration for the Chamber Music Series (when offered) and/or the Concerto Competition.

2017-2018 Tuition Assistance Request Form

Please contact the Youth Symphony of Southern Oregon (info@ysso.org) to request a copy of this form. In order to be considered, the tuition assistance request form must be completed and turned in with all completed, signed registration/release forms at registration. Please review the instructions and provide all of the information requested.

If the annual required registration/release forms are not turned in at registration, musicians and families must complete, sign and mail all forms and tuition/fee payments to the YSSO, P.O. Box 4291, Medford, OR 97501, by no later than October 9.

All musicians must be registered and all completed, signed forms must be turned in each year.

NOTE: The YSSO uses photographs, reproductions, recordings, and other materials for publicity and promotional purposes and online. All YSSO musicians and families must complete and sign the Media, Recording & Photo Release and submit the form with all other annual registration/release forms.
